The Robert Wood Johnson Foundation (RWJF) seeks to help improve community health by fostering connections across systems that are built to last. If different sectors have the capacity and urgency to work together to address community needs, they can make and sustain progress.

Efforts to increase cross-sector alignment are well underway. This call for proposals supports RWJF’s intent to identify and share what works to effectively align the health care, public health, and social services sectors.

Required Components

Aligning Systems for Health will explore the degree to which health equity is impacted or results from current models of collaboration incorporating health care, public health, and social services. Gaps in health are large, persistent and increasing, and RWJF is committed to a system that meets people’s goals and needs and addresses these gaps that many populations face. Studies should include a focus on health equity by examining how cross-sector alignment can advance the opportunity for everyone to have a fair and just opportunity to be as healthy as possible. RWJF will offer and encourage awardees to take advantage of opportunities to build equity as a core practice within the first six months of the grant period.

A key role of Aligning Systems for Health is turning evidence into action. Grantees will be expected to participate actively both in research dissemination and translation mechanisms organized by GHPC and RWJF, including blogs, podcasts, research meetings, and policy briefings, among others.

Alignment Strategy

Eligibility and Selection Criteria

  • Preference will be given to applicants that are either public entities or nonprofit organizations that are tax-exempt under Section 501(c)(3) of the Internal Revenue Code and are not private foundations or Type III supporting organizations. The Foundation may require additional documentation.
  • Applicant organizations must be based in the United States or its territories.

    Phase 2 full proposal submission begins for selected Phase 1 applicants.

    Deadline for receipt of full proposals for selected Phase 1 applicants.
     
  • Early March 2020
    Applicant finalists notified of funding recommendations.
     
  • May 1, 2020
    Grants initiated.

Total Awards

  • Up to $2 million will be available under this CFP.
  • Project funding can be up to $400,000 to accommodate projects up to 24 months.
  • Up to 12 grants will be funded.
