The Coalition Self-Assessment Survey was developed as a tool for coalitions to obtain feedback about how members of a collaborative project perceive that a coalition is doing. The survey data provide valuable information about aspects of the coalition that might need improving. Any statewide or local coalition, partnership, consortium or collaborative group working together on a community-level public health, health care or social service problem could benefit from using the survey. This manual describes how the survey can be customized; who should be included in the sampling frame and how a sample should be drawn; how and to whom the results should be presented; and what structures and processes need to be in place to apply the results appropriately. The Survey Instrument developed originally for the SmokeLess States®: National Tobacco Policy Initiative is included as part of the appendices. A review of the content of the survey explains not only what is in the survey but why it is in there. The authors hope that this manual will help other coalitions to use the self-assessment survey to measure their activities and ultimately get the results that they need.
