Aligning Forces for Quality: Snapshots of Success

Aligning Forces for Quality (AF4Q) tests innovative strategies for improving health care at the community level. With a focus on performance measurement and public reporting, quality improvement interventions, and consumer engagement, these communities are developing unique approaches to address the health care challenges in their regions. These snapshots of success highlight one activity from each of the original 14 AF4Q communities and outline the steps involved, lessons learned and results of the activity. Some highlights include:

  • A Primary Care Renewal Collaborative in Humboldt County, Calif. that is saving lives by redesigning the way medical practices deliver care
  • The “I Can! Challenge” social marketing campaign in South Central Pennsylvania that inspired individuals to better manage their health and partner with their doctors
  • The Ask Me 3 health literacy program in Wisconsin that increased patient satisfaction by helping individuals learn their role in their care
  • A radio partnership in Cincinnati, Ohio, that reached 90 percent of the African American community with information on a new health care quality report and how to get good diabetes care
  • A toolkit that employers used to give their employees information on the performance of physician organizations in southeast Michigan

AF4Q brings an unprecedented commitment of resources, expertise and training to turn proven practices into real results on the ground. Operating in 15 communities, AF4Q is teaming up those who get care, give care and pay for care to deliver lasting change.
