New Coordinated Care Model Shows Promise for Chronically Ill Children

From 1995 to 2000, staff at Children's Hospital in Boston planned and implemented the Pediatric Alliance for Coordinated Care, a clinical service delivery model that facilitates coordinated, community-based, family-centered care for children with special health care needs who are enrolled in Medicaid in Massachusetts.

The project was part of the Robert Wood Johnson Foundation (RWJF) Building Health Systems for People with Chronic Illnesses national program.

Key Results

  • As of the end of 2000, the Pediatric Alliance for Coordinated Care, formed as a consortium of six pediatric practices, was serving more than 150 children.
  • The alliance did not complete a planned financing structure for managed care, due to the difficulties in obtaining and analyzing necessary data.

Funding
RWJF supported this project through two grants, for planning and implementation, totaling $660,061.
