From 1995 to 2000, staff at Children's Hospital in Boston planned and implemented the Pediatric Alliance for Coordinated Care, a clinical service delivery model that facilitates coordinated, community-based, family-centered care for children with special health care needs who are enrolled in Medicaid in Massachusetts.
The project was part of the Robert Wood Johnson Foundation (RWJF) Building Health Systems for People with Chronic Illnesses national program.
