Partnership Streamlines Care for Families with Disabled Children in Rural Missouri

From 1998 to 2002, staff at University of Missouri's School of Medicine developed and implemented the Missouri Partnership for Enhanced Delivery of Services (MO-PEDS).

The program is a comprehensive, family-centered, community-based health delivery system designed to meet the needs of children with special health care needs who are classified as permanently and totally disabled.

The project was part of the Robert Wood Johnson Foundation (RWJF) Building Health Systems for People with Chronic Illnesses national program.

Key Results

  • As of October 2000, 91 families were enrolled in MO-PEDS.
  • Communication with families was strengthened via a Web site that helped link families to providers, a newsletter and a toll-free telephone number.
  • Project staff designed one half-day workshop and several in-service presentations to help primary care physicians identify children with special health care needs and to improve primary care service delivery to this population.
  • Project staff designed an evaluation plan to measure child and family outcomes, utilization, and other changes in the health care system as a result of the MO-PEDS project.

RWJF supported this project through a grant of $450,942.
