The Robert Wood Johnson Foundation Anthology
series is a collaborative effort, and we owe a debt of gratitude
to the many people who have worked with us in the editing
and production of Volume VIII. Within the Robert Wood Johnson
Foundation, David Morse, especially, has been an invaluable
partner. An active participant from beginning to end, he
has brought thoughtfulness, sound judgment, and creativity
to the project. In addition to writing the foreword, Risa
Lavizzo-Mourey reviewed all the chapters and made valuable
suggestions for their improvement. Ayorkor Gaba did an outstanding
job of pulling together materials and helping us in a variety
of other ways, as did her successor, Katherine Muessig. Molly
McKaughan’s editorial review of the chapters led to
improvement in all of them. Deborah Malloy, Nancy Giordano,
Sara Wilkinson, and Sherry De Marchi made it possible for
communications between San Francisco and Princeton to flow
smoothly. Paul Moran, Tim Crowley, Carol Owle, Mary Castria,
Carolyn Scholer, Ellen Coyote, and Russ Henshaw handled financial
transactions with their usual efficiency. Hope Woodhead oversaw
distribution matters impeccably, and Barbara Sherwood made
sure the book was always displayed in the Foundation’s
lobby and got to the right people outside of the Foundation.
Richard Toth and Lydia Ryba checked the accuracy of the dollar
amounts and dates of all Foundation grants mentioned in the
book. Hinda Feige Greenberg, Katherine Flatley, and Mary
Beth Kren were of great help in identifying and finding material
needed by editors and authors alike.
Beyond the Foundation, C. P. Crow again
demonstrated why he is considered a legendary editor. Carolyn
Shea continued to be an extraordinary fact checker. Lauren
MacIntyre converted hand-marked edited copy into printed
text with amazing speed and accuracy. At Health Policy Associates,
Elizabeth Dawson was a trusted colleague in all aspects of
editing and producing the Anthology and Greta McKinney performed
her bookkeeping duties conscientiously, as usual.

chapters, and we wish to acknowledge their contribution.
