Quality Improvement Intervention through Health Departments

Participation in QI team development and onsite QI training through the Community Outreach and Change for Diabetes Management (COACH 4DM) improved LHD outreach and delivery of diabetes education.

The Issue:
Local health departments (LHDs) must continually improve public health services. Quality improvement (QI) techniques are effective in helping LHDs improve the delivery of services. This study examines the impact of QI on LHDs’ delivery and outreach of diabetes self-management education (DSME) services.

Key Findings

  • There was a greater than 100 percent increase in the number of individual completing an entire course of DSME.

  • Half of LHDs reported receiving increased numbers of referrals per month.

About the Study:
The COACH 4DM model was delivered to six LHDs providing DSME during 2010-2011. LHDs then development and implemented a QI project to improve the outreach and delivery of DSME services. Pre and post-intervention surveys evaluated the change in DSME outreach and delivery.
