Aligning Forces for Quality, a national program of the Robert Wood Johnson Foundation, developed this checklist as a guide to help consumers prepare for medical visits.

When getting ready for a visit with a medical professional, consider these recommendations:

  • Prepare questions in advance of the appointment.
  • Ask questions about your treatment, condition or self-management.
  • Talk freely about prescriptions or medications you are taking, symptoms, treatments and test results.
  • Take notes during the visit.
