Building a Community Quality Report at the Physician Practice Level

The Experience of the Puget Sound Health Alliance

In January 2008, the Puget Sound Health Alliance (the Alliance) released the Community Checkup, its first public report containing quality measures for physician clinics in the area. The report was the culmination of a process that began before the Alliance became an Aligning Forces for Quality (AF4Q) grantee and involved broad stakeholder representation.

This evaluation brief describes the key features of that process, including preparing the report, content, format, dissemination and key lessons learned by the Alliance.
