Synopsis of Work: The Medicare/Medicaid Integration Program (MMIP) (1997 to 2006) was a state demonstration program that tested the operation and design of delivery systems that integrate long-term and acute care services under combined Medicare and Medicaid capitation payments for elderly patients.

Story Told: Estella set a potholder on fire because she could no longer read the burner knobs on her stove to tell if they were "on" or "off." A nursing home seemed like the only way to keep the 86-year-old woman safe.

Estella's care coordinator in Minnesota Senior Health Options pursued a different plan.

She assessed Estella's condition and needs, then presented Estella a care plan that would permit her to stay in her home.
