When Getting There Isn't Half the Fun: Improving Transfers of Patients
Invitational conference on transitional care
In 2002, the University of Colorado Health Sciences Center hosted a conference to develop research and policy agendas for improving the quality of care for persons who are transferred from one medical setting to another.
Key Results
- The Aspen Transitional Care Conference, which took place September 18–20, 2002, in Aspen, Colo., drew 39 national experts in the field of transitional care. Topics included:
- Advancing research.
- Enhancing patient and caregiver roles and skills.
- Implementing greater accountability through performance measurement.
- Aligning financial incentives and reimbursement.
- Ensuring timely and accurate information transfer.
