When Getting There Isn't Half the Fun: Improving Transfers of Patients

In 2002, the University of Colorado Anschutz Medical Campus hosted a conference to develop research and policy agendas for improving the quality of care for persons who are transferred from one medical setting to another.

Key Results

  • The Aspen Transitional Care Conference, which took place September 18–20, 2002, in Aspen, Colo., drew 39 national experts in the field of transitional care. Topics included:
    • Advancing research.
    • Enhancing patient and caregiver roles and skills.
    • Implementing greater accountability through performance measurement.
    • Aligning financial incentives and reimbursement.
    • Ensuring timely and accurate information transfer.
