Martha's Vineyard - A Model for Health Reform?
Developing a community-based health system on Martha's Vineyard
From 2000 to 2001, the Foundation for Island Health developed a business plan and continued work toward a pilot community-based health care system on Martha's Vineyard.
Key Results
- Project staff established the Youth Health Project to develop, in collaboration with students and parents, a curriculum that would promote healthy behavior among teens.
- Project staff initiated the Coordination of Care Project, through which two visiting nurse groups addressed issues of medication compliance, side effects, pain control, education and psychosocial support for patients and their families.
Funding
The Robert Wood Johnson Foundation (RWJF) supported this project through a grant of $50,000.
