The mission of the Robert Wood Johnson Foundation Community Health Leaders (CHR) program is to support and sustain the capacity of individuals who demonstrate courage, creativity, and commitment to improve health outcomes at the community level, with a special focus on underserved populations. To achieve the goal and carry out the mission, the CHR's National Program Office (NPO) conducts initiatives to bring visibility to the previously chosen leaders and their work, organizes and supports opportunities for leaders to learn new skills and develop new resources, and disseminates lessons learned by the Community Health Leaders. Over the past 20 years, the Foundation has awarded 206 leaders, up to 10 leaders each year, through a national nomination and selection process (including one special Gulf Coast solicitation). The NPO also gathers census information about the leaders; supports networking and interactive communication among the leaders; conducts enrichment and skill building activities; and supports opportunities for leaders to develop new relationships and resources with a broad array of field experts. This six-month technical assistance and direction grant supports continued program monitoring and support/technical assistance and related activities for all previously named leaders, particularly the 2012 leaders, virtual convenings of a subcommittee of leaders to help inform 20th anniversary activities, convening the NAC, and personnel, office space/supplies and other NPO outreach activities.
