The Foundation's initiative, Mobilizing Action Toward Community Health (MATCH), was designed to help communities translate the County Health Rankings into multisector action that addresses the social, economic, environmental and behavioral factors that affect health.This contract funds the support of consultant Kitty Jerome for the Roadmaps to Health Prize program, which will elevate examples of strong community leadership and messages from the Robert Wood Johnson Foundation (RWJF) and the County Health Rankings & Roadmaps program on the importance of cross-sector collaboration in addressing the multiple determinants of health. The Prize, a collaborative effort of the Foundation and University of Wisconsin Population Health Institute (UWPHI), will be awarded to six communities in early 2013 for their outstanding initiatives in communitywide health improvement. The Foundation and UWPHI will host a recognition event, support local events, and capture and share the stories and lessons of the winners through media, social media, in-person meetings, conferences, and events. As deliverables, Jerome will: (1) serve as primary liaison, on behalf of RWJF and UWPHI, with the winners and finalists to prepare them for the prize announcement and post-announcement outreach activities, and (2) develop a logistics guide for the recognition and promotional events, a survey of prize winners about the post-announcement experience, and recommendations on developing a speakers bureau and networking opportunities for the winners.
