The Foundation's program, New Jersey Health Initiatives, was designed to support projects in New Jersey that develop new approaches to resolving the state's health care needs, focusing on the Foundation's goal areas.The Hunterdon Regional Community Health is the lead organization within the Hunterdon Healthcare System and will collaborate with Hunterdon Medical Center, the Hunterdon County Health Department, and 30+ organizational members of the Partnership for Health in Hunterdon County in this grant. They propose a more efficient model of community assessments and implementation of the 'CHIP' through linking multiple assessments and secondary data online through a public health information exchange (data sharing repository) and implementation software. This software allows organizations to construct 'Strategy Maps' and 'Community Balanced Scorecards' to facilitate development and execution of strategic goals, measurable objectives and deliverables. The Partnership for Health website will also house Web 2.0 applications including a community Wiki, health education blogs along with interface to Facebook and LinkedIn networks. Social marketing strategies will be used to promote professional and public use. To evaluate the effectiveness, a pre-post organizational network analysis will be done to determine whether improvements were made in: (1) increasing membership to Partnership for Health; and (2) building/strengthening organizational partnerships.
