Expanding Coming Home Connection's volunteer model of in-home personal and bedside care for individuals with chronic or terminal illness, 2011-2013
The Foundation's Local Initiative Funding Partners Program is a matching grants program designed to establish partnerships between the Foundation and local grantmakers in support of innovative, community-based projects that improve health and health care for underserved and vulnerable populations.Through the Coming Home Connection, low-income children and adults with severe chronic or terminal illness receive free, in-home, long-term care provided by trained volunteers. Up to 24 hours per day personal and bedside care provides respite for family members and fills gaps when no coverage is available from Medicaid, Medicare, insurance and hospice. Program expansion from Santa Fe to additional rural northern New Mexico counties includes outreach to veterans who may become volunteers or clients. Half of the volunteers in the pilot project were health professionals including nurses, nursing students, physical therapists, massage therapists and personal trainers.Local funding partners include: Con Alma Health Foundation, Inc.; Blue Cross Blue Shield of New Mexico; The Coker Foundation; Daniels Fund; Sydney & Andrew Davis Foundation; The Foundation for Hospice Care; McCune Charitable Foundation; New Mexico Community Foundation; PMS Hospice Center; and the Still Point Fund.
