Robert Wood Johnson Foundation Community Health Leaders
The Robert Wood Johnson Foundation Community Health Leaders program was designed to provide recognition for the contributions community health leaders make to achieving RWJF's mission and goals and to enhance the capacity of these individuals to have a more permanent and widespread impact on health care problems.The program annually honors 10 individuals who have made significant contributions to improving the health and health care of their communities. Each Community Health Leader receives an award in the amount of $125,000. Twenty thousand dollars goes directly to the leader to recognize past accomplishments and $105,000 goes to the leader's organization to support the continuation of and enhancement of the leader's work. This award supports the Family Support Services of West Hawaii, the organizational home of 2008 Community Health Leader, Deann Canuteson.The goal of this project is to develop a strategic plan to establish a Family Resource and Referral Center in the district of Kau, Hawaiian Ocean View Estates. The center is envisioned as a vehicle through which residents of this rural community will receive timely and accurate health-related information and guidance to the appropriate providers who can meet their health care needs.During the project period, staff intend to complete a strength and needs assessment which will include people with special needs or developmental disabilities as a subcategory. The assessment will serve as a baseline for the future development of services in the Kau district. Another project objective is to identify funding sources for meeting the needs that are documented. Key project activities will include meetings with individual stakeholders as well as community groups to gather input for the plan and to prepare a report which documents the findings. The report will be distributed to constituents and decision makers for further action.
