Evaluation of RWJF's Aligning Forces for Quality program

The Foundation's Regional Market Plan initiative was designed to support the first step in an effort to accelerate improvements in care at the community level by cultivating and aligning market forces with quality improvement efforts. The purpose of this grant is to fund the comprehensive four-year evaluation of the Foundation's Aligning Forces for Quality Program. The evaluation has been planned from the inception of the Program. Fourteen Aligning Forces regions have received awards, and it is time for baseline measurement of regional experience with quality of outpatient chronic illness care. The evaluation will provide three types of information: (1) regional insights about the regions at baseline that can be used for planning each region's approach; (2) ongoing monitoring information to fine-tune the program; and (3) evaluation of "outcomes" (i.e., clinical outcomes, provider practice change outcomes, and consumer engagement related outcomes). Deliverables include: (1) site level reports describing the functioning of each regional coalition as it unfolds, (2) tracking information on the development of performance measurement/public reporting systems; and (3) baseline and post-implementation assessments of consumer experience, changes in outpatient care, outcomes and cost. In addition, there will be collaboration with individual regions on the evaluation of their specific approach.
