Common Ground: Transforming Public Health Information Systems
The Foundation's program, Common Ground: Transforming Public Health Information Systems, was designed as collaborative processes among state and local public health departments to advance the use of information systems to support two different programmatic areas--preparedness and chronic disease.The Missouri Department of Health and Senior Services will redesign its chronic disease systems by constantly striving to improve its information systems. The project team will identify common elements of chronic disease systems for all public health programs, use quality improvement methods to improve program outcomes and enhance data collection systems for chronic disease surveillance. The grantee will train and engage internal and external chronic disease stakeholders in these efforts to prevent and control chronic disease conditions at the local community level.
