Common Ground: Transforming Public Health Information Systems
The Foundation's program, Common Ground: Transforming Public Health Information Systems, was designed as collaborative processes among state and local public health departments to advance the use of information systems to support two different programmatic areas--preparedness and chronic disease.The Coconino County Health Department (CCHD), a very large rural health department in northern Arizona, provides a very broad scope of services and supports a staff of 130 members in a county with frontier population status. CCHD faces the challenge of multiple jurisdictions, including six incorporated cities and towns (one of which is shared with another county, and one of which is on a sovereign Indian nation), six Native American tribes in whole or in part, and federal and state agencies. The purpose of this project is for CCHD to develop an integrated information system for the Department through the Common Ground business analysis process for their chronic disease programs.
