Common Ground: Transforming Public Health Information Systems

The Foundation's program, Common Ground: Transforming Public Health Information Systems, was designed as collaborative processes among state and local public health departments to advance the use of information systems to support two different programmatic areas--preparedness and chronic disease.The Minnesota Common Ground project seeks to improve the health of individuals and communities in Minnesota by improving the capacity of information systems to support disease prevention, disease and risk factor surveillance, and chronic disease management in all populations. The purpose of this project is for the grantee to identify business processes, information needs, data flows and opportunities for improved data use. They will share information system approaches and tools across programs, and work toward greater interoperability and integration where appropriate. Minnesota has a substantial foundation of leadership commitment, skills, experience and governance structures to effectively support this project.
