The purpose of this project is to package self-management products into multimedia toolkits that guide leaders in their implementation and create a marketing strategy for disseminating these self-management products. The two audiences of leaders are rural primary care center staff and lay leaders from churches, diabetes coalitions, and outreach organizations.
