Investigating up to ten more communities for a potential role in the pilot for RWJF's Aligning Forces for Quality project
The Foundation's Regional Market Pilot is a set of quality improvement activities in a specific community designed to help align quality improvement efforts. The Foundation has entered into a contractual agreement with the Center for Health Improvement to perform the initial six-month phase of work for this pilot project. The Center evaluated quality improvement efforts in four communities (Minneapolis, Detroit, Seattle, and Memphis). This project will investigate up to ten more communities for a potential role in the pilot. Some of these additional communities will be smaller metropolitan areas (e.g., population less than 500,000) than the initial four. Staff will scan additional communities to provide more options for implementing the pilot. Smaller communities may allow the project's actual intervention to have a more discrete, definable impact. This pilot project, in turn, will be critical to the success of the Foundation's strategic objective to "accelerate performance improvement on nationally adopted measures of outpatient chronic care quality through local market demonstrations." The scanning of these additional communities will also be done by the Center for Health Improvement.
