Supporting activities for a health plan collaborative to reduce health care disparities

The National Health Plan Collaborative comprises eleven major health insurance plans that work in partnership with organizations from the public and private sectors to identify ways to improve the quality of health care for racially and ethnically diverse populations. Each of the Collaborative’s members uses race and ethnicity data to identify disparities in care and evaluate the effectiveness of provider-, member- and community-targeted interventions aimed at reducing gaps in care and improving the quality of care minority populations receive.
