Best practices for end-of-life care in nursing homes

About 25 percent of all deaths in America now occur in nursing homes, which represents a rapid increase in recent years. The capacity of nursing homes to care for dying patients is greatly limited. This grant provides support for the integration of hospice care in nursing homes by identifying best practices to overcome administrative barriers to hospice and nursing home collaboration. The project deliverable will be a manual that includes templates and tools to help hospices and nursing homes iron out administrative issues pertaining to billing, staffing, and communication. This project will be considered successful if it produces a product that is adopted and used by nursing homes, hospices, and their associations.
