Systemic changes to achieve a continuum of mental health and substance abuse services

The Foundation's Local Initiative Funding Partners Program is a matching grants program designed to establish partnerships between the Foundation and local grantmakers in support of innovative, community-based projects that improve health and health care for underserved and vulnerable populations.This grant provides continued support for this public-private partnership which engages approximately 40 mental health and 40 substance abuse providers to cooperate in creating a single, more-efficient care and referral system, more easily accessed by people in need. After an initial planning year, these diverse community agencies are well on the way to systemic change, increasing both communication and collaboration throughout Larimer County, Colorado. A new Connection Center, open 24/7, including a telephone hotline, provides an enhanced information and referral system where patients can receive immediate help. The restructured continuum of care now bridges mental health and substance abuse programs and includes access points such as the police and public schools. The community expects increased capacity from more efficient use of available services; improved responsiveness; less waiting; and more appropriate diagnosis, treatment, and aftercare.
