Establishing a community support team for elderly, chronically ill residents following inpatient hospitalization

The Foundation's program, New Jersey Health Initiatives, was designed to support innovative community-based projects that address one or more of the Foundation's goals in health and health care.The purpose of this project is to address the well being of frail, elderly, isolated individuals following an outpatient hospitalization. The Community Support Team, in collaboration with the AtlantiCare Health System and the Atlantic County Department of Intergenerational Services, will address improving access to basic health services, improving care and support for people with chronic health conditions, and the promotion of healthy communities and lifestyles. Staff will follow participants for an average of six months by phoning participants at a minimum of twice monthly to monitor their well being. The Team will also address the systemic issues that increasingly affect the lives of elderly people who have no local support systems when they come home from the hospital.
