Intervention and education program for Hispanic hospice patients and their primary caregivers
The Foundation's Local Initiative Funding Partners Program is a matching grants program designed to establish partnerships between the RWJF and local grant makers in support of innovative, community-based projects that improve health and health care for underserved and vulnerable populations.The Hispanic community in Boca Raton, Florida, has grown to over 400,000 individuals, yet very few avail themselves of well established local end-of-life services. The purpose of this project is to: (1) recruit a culturally specialized interdisciplinary team to serve Hispanic people with life-limiting illnesses; (2) partner with Hispanic community-based organizations to educate patients and families about these quality palliative care resources; and (3) implement a cultural competency learning program for all 400 of the agency's hospice and home care workers. This learning program about end-of-life care will also be offered to other health care providers and is designed to be the first phase of a long-range effort to reach out to additional target populations including African-Americans and Haitians in the area.
