The purpose of this project is to create a working group of heart failure specialists, palliative care specialists, and peer review organizations (PROs) to address supportive care in heart failure. To date, there has been no discussion or consensus in the field on a palliative care approach to the treatment of heart failure. This project will convene a two-day consensus conference with experts in heart failure, quality-of-life research in heart disease, and palliative care to identify gaps in current knowledge, practice, and research relating to symptom management and holistic support of patients and families at high likelihood of death from heart failure. The work group will collaborate with heart failure treatment and research centers and PROs to: (1) obtain funding from another source to design and implement a trial of supportive care for patients with advanced heart failure; and (2) create and implement patient, family, and health care provider education to address management of symptoms, functional problems, and social issues, including advance planning for management of dying or sudden death.
