The purpose of this project, using peer-based health promoters to reach patients with asthma, diabetes, and hypertension, is to affect systems change from provider-centered care to patient-centered care in a rural area of Puerto Rico. This health care agency has a successful track record of program delivery and sustainability, and replication in other health centers is expected.
