Peer-based effort to improve health care for people with chronic conditions through education and self-management

The Foundation's Local Initiative Funding Partners Program, a matching grants program, was designed to establish partnerships between the RWJF and local grant makers in support of innovative, community-based projects that improve health and health care for underserved and vulnerable populations. The purpose of this project, using peer-based health promoters to reach patients with asthma, diabetes, and hypertension, is to affect systems change from provider-centered care to patient-centered care in a rural area of Puerto Rico. This health care agency has a successful track record of program delivery and sustainability, and replication in other health centers is expected. The funding partner is The Puerto Rico Community Foundation.
