Comprehensive health services program for homeless people
The Foundation's Local Initiative Funding Partners Program is a matching grants program designed to establish partnerships between the Foundation and local grantmakers in support of innovative, community-based projects that improve health and health care for underserved and vulnerable populations.Health care for the Homeless provides a continuum of care for the homeless people of Savannah, Georgia. By creating a 32-bed respite center, the project will facilitate timely discharge of homeless people from hospitals and enhance their recuperation. The center is expected to have a 90 percent occupancy rate and will be open 24 hours, 365 days per year. The new respite center will also house primary care, dental, and mental health services, thereby directing homeless people away from emergency rooms for primary and preventive care. Funding partners include Memorial Health System, St. Joseph's Candler Health Systems, and Chatham-Savannah Authority for the Homeless.
