Helping primary care practitioners assess genetic vulnerability to common chronic diseases

This grant provides support for the development and evaluation of a Web-based approach to obtaining and interpreting complete family histories, particularly for common chronic conditions; developing evidence-based algorithms to assist in evaluating and addressing individual patient's risk of disease; and designing and conducting a clinical trial to determine effectiveness of using the Web-based history collection in conjunction with the algorithms from both the practitioner and patient perspectives. This project is expected to result in a user-friendly electronic tool, tested in terms of precision and usefulness, which can assist patients and practitioners to have better medical histories in context with current science-based evidence, and be readily applicable in a large-scale demonstration project.
