The purpose of this project, which consists of a planning meeting and two invitational conferences, is to bring together experts and researchers on medical self-care, managed care representatives, and health professional organizations to identify research gaps, gaps in the translation of research to practice, and recommended future directions for diffusion of medical self-care in managed care. Specific outcomes of the conferences will include: (1) review of current knowledge about medical self-care and older persons, including issues in the design and evaluation of self-care programs; (2) review of managed care delivery of primary care to chronically ill older adults, including strategies such as patient education, care management, cooperative care, etc.; (3) identification of research and practice gaps; (4) identification of high priority areas to use in future program development activities (in research and practice); and (5) recommendations that might promote integration of self-management of chronic disease in managed care settings and structures. A product of these conferences will be a monograph highlighting the innovations in integrating medical self-care and managed care and an agenda for future research and program development.
