The Foundation's program, Improving Chronic Illness Care, was designed to help health care organizations redesign care to improve the clinical and functional outcomes of patients with chronic illness. The program seeks to support research to increase understanding of the interventions and health system changes that favorably affect chronic illness outcomes.This grant supports start-up activities for the program. The National Program Office will begin recruitment for project personnel, including an Associate Director for the National Chronic Disease Center; develop plans for the first Breakthrough Series (BTS), in collaboration with the Institute for Healthcare Improvement; develop a process for selecting candidates for a research team to evaluate the BTS; and convene meetings and conduct conference calls to build networks with national organizations involved in the program's three target areas: managed care organizations, safety net providers, and academic practices.
