Outreach program to address the health and social service needs of homeless persons
The Foundation's program, New Jersey Health Initiatives, was designed to support projects in New Jersey that develop new approaches to resolving the state's health care needs, focusing on the Foundation's goal areas.This Project Hope Street Outreach to the Homeless project employs a mobile outreach team to provide health services, referrals, and case management to homeless individuals living on the streets of Camden. This team consists of a nurse and two community outreach workers, both of whom have personally experienced homelessness. They will reach out to homeless individuals and families, helping them make connections with existing health and social services. Rutgers University School of Social Work will evaluate the project.
