The Foundation's program, New Jersey Health Initiatives, was designed to support projects in New Jersey that develop new approaches to resolving the state's health care needs, focusing on the Foundation's goal areas.Through the collaborative efforts of a social service provider and a health care center, this project will establish a clinic for people with developmental disabilities at the Kimball Manchester Ambulatory Center in Whiting, New Jersey. Open to all individuals with developmental disabilities (county residency is not an eligibility requirement), this clinic will provide primary care and emergency services in a coordinated manner, using a central medical record registry. Services include medical, dental, psychiatric, and social evaluations, referral to specialists as needed, and case management.
