The objectives are to promote integration of acute and long-term care services in managed care plans, create health plan accountability for costs and outcomes, and reduce incentives for cost shifting between Medicare and Medicaid. The purpose of this project is for the grantee to continue to develop and implement a program to reform the system of long-term care financing and service delivery in the State of Florida.
