Planning a managed care system for children with severe chronic disorders
The Foundation's initiative, Building Health Systems for People with Chronic Illnesses, was designed to find models of caring for people with chronic illnesses that will overcome the fragmentation, financing barriers, and episodic care of the current system.This is a one-year planning grant to develop a full-service, Medicaid-funded, managed care system for children with severe chronic illnesses and disabilities. The primary objectives are to define costs of the target population, establish capitation rates, develop a primary care physician network, design a clinical model, build a network of community providers under contract with Pediatric Community Alliance, and create a payment formula. A quality monitoring system and marketing strategy will also be developed.
