Development of a family health maintenance organization
Under this grant, the Contra Costa County Health Plan, a publicly run HMO with a mixed income clientele, will complete the design of an enriched HMO program for high-need families with complex health problems. The plan is operated by the County Department of Health which has jurisdiction over public health, substance abuse, mental health, Medicaid, and family support services. Also included will be services funded by other county agencies such as social services, employment and training, child protective services, and the court system. The family HMO will develop capitated financing for enrolled families and participation will be made available to anyone receiving services from the Health Department. Marketing materials will be prepared and distributed in late 1994. Additional funding may be requested for the implementation phase of the project.
