Pilot service program for high-risk, high-cost Medicaid patients

Under the first phase of this project, the State of Maryland will undertake the design and pilot testing of a system for identifying, managing, and monitoring high-risk, high-cost Medicaid patients. The goals of this project are to, within a state Medicaid population: (1) reduce preventable hospital admissions, readmissions, nursing home admissions, and emergency room visits; (2) provide care in the lowest-cost setting appropriate; (3) reduce average annual total costs per Medicaid recipient; and (4) reward providers who can deliver quality care efficiently to high-risk, high-cost patients.
