Integrating financing and services for disabled persons in California

This grant provides support to develop a service financing model in housing for homeless, chronically disabled single adults in three urban northern California counties. The purpose of the project is to increase the effectiveness of the service systems linked to housing and thereby prevent the revolving institutionalization and recurrent homelessness of tenants in such housing. The target population consists of people with multiple, serious problems, including schizophrenia, drug abuse, AIDS, etc. Therefore, the model is expected to integrate funding for a whole range of services, including mental health, substance abuse, social services, primary care, jail diversion, and employment. The grantee may return for implementation funding upon successful completion of the planning phase.
