Monroe County child health initiative

In July 1991, the Foundation authorized an increase in the number of sites under the Child Health Initiative from four to ten. It was noted that several communities, including Rochester, expressed an interest in submitting a proposal. The Monroe County Health Department, in cooperation with the Department of Pediatrics of the University of Rochester School of Medicine and Dentistry, will focus to achieve funding and systems changes to make human services more comprehensive and responsive to the needs of birth-school age, high-risk children. The project will also pursue one or more Medicaid maximization strategies including targeted case management. The project intends to merge the maternal and child home visitation, outreach, tracking and care coordination programs administered by the Monroe County Health Department. Simultaneously, the project will be working with the county's Department of Social Services and other agencies to incorporate other program funding sources into the Initiative.
