Partners in Caregiving: The Dementia Services Program
The Foundation's Partners in Caregiving: The Dementia Services Program was designed to provide technical assistance and grant support to help adult day centers develop and strengthen innovative center-based, in-home, and other respite programs for people with chronic cognitive disorders--particularly dementia. The purpose of this project is to develop a dementia care day center which will serve up to 18 clients per day, 6 days a week. In addition to day care, they will provide an in-home companion service, offering hourly and round-the-clock services, and early morning/late afternoon services in assistance of the day care. A provision of institutional short-term respite services will also be available. The Nevins Family of Services plans to conduct bi-monthly support groups for caregivers as well as training programs for families on utilization of collateral programs offered such as contracted hospice, home management, and care management. It is anticipated that the program will become financially self-sufficient by the end of the three-year grant period.
