Development of case management system for serious disorders

This grant supports the development of a case management approach to the care of patients with congestive heart failure, diabetes, and hypertension. It builds upon and extends the work of the Stanford Cardiac Rehabilitation Program called MULTIFIT. MULTIFIT is a federally supported multifactorial risk intervention project that relies on trained nurses, supported by computer databases, to manage coronary risk factors in the year after acute MI. The principal objectives of the project are: (1) locate and compile existing practice guidelines for the management of congestive heart failure, diabetes, and hypertension; (2) analyze current treatment practices for these conditions through medical record review; (3) develop preliminary management algorithms; (4) pilot test approximately 100 patients in each of the 3 disease categories; (5) develop a training course for nurses; and (6) develop a computer-based expert system for dietary counseling. Subsequently, the grantee plans to test the efficacy of the system through a multisite federally supported demonstration.
