Rural case management program for pregnant and parenting teens

The Foundation's Local Initiative Funding Partners Program is a matching grants program designed to establish partnerships between the Foundation and local grantmakers in support of innovative, community-based projects that improve health and health care for underserved and vulnerable populations.This grant provides continued support for a rural comprehensive case management program for pregnant and parenting adolescents and their families in Southwest Idaho under the Local Initiative Funding Partners Program. This additional support will increase to 330 the number of clients enrolled and provide data on 108 mothers who are one year postpartum and 71 mothers who are two years postpartum. Data will be collected on 441 WIC clients in the Ada County comparison group. If long-term benefits can be documented, the cost effectiveness of the project can be established. The objectives of the program are to decrease the incidence of low birth weight infants, increase school continuation, decrease or postpone repeat births, and increase employment opportunities.
