Specialized Residential Health Care Facility Program
The Foundation's program, New Jersey Health Initiatives, was designed to support projects in New Jersey that develop new approaches to resolving the state's health care needs, focusing on the Foundation's goal areas.The Residential Services Unit of the New Jersey Department of Human Services has primary responsibility for overall planning and policy development related to board and care housing facilities. They will establish a new and cost-efficient level of residential care for 20-25 young adults with severe and persistent problems of mental illness. The Specialized Residential Health Care Facility (SRHCF) will combine low-cost housing, coordinated health and social service care under the direction of a case manager, and 24-hour supervision at a demonstration site. With the establishment of the facility, the project will: (1) establish a working model for SRHCFs which can be self-sustaining without Foundation funds and cost-efficient compared to other service-enriched housing alternatives for this population; and (2) if successful, replicate the project in two additional sites.
