Improving Chronic Illness Care (ICIC) is a national program dedicated to improving the quality of life among the 133-million Americans who suffer from diabetes, depression and other chronic conditions. For a decade, ICIC has worked with national partners, funded studies and provided technical assistance to help health systems--especially those that serve low-income populations-- amend their care through implementation of the Chronic Care Model. The Chronic Care Model identifies the essential elements of a health care system that encourage high-quality chronic disease care, including the community, the health system, self-management support, delivery system design, decision support and clinical information systems.
