Studying accountable care organizations at full scale to test viability

The Foundation's initiative, Accountable Care Organizations: Testing Their Impact, was designed to study the effect of accountable care organizations (ACOs) on a variety of quality-related outcomes and to conduct research analyses that will help the field better understand how ACOs will affect health care.This grant provides funds to Providence Portland Medical Center to conduct case studies of two coordinated care organizations (CCOs), one urban, one rural, in Oregon. CCOs are Oregon-specific accountable care organizations (ACOs) that serve Medicaid patients. This funding is for one of four sets of case studies designed to enhance the Robert Wood Johnson Foundation's understanding of ACOs that are developing in the private market, with a special emphasis on ACOs that assume downside risk and that care for patients in the safety net. The case studies will focus on how the two CCOs are organizing themselves to produce better care, better health, reduced disparities, and lower costs, and on how they manage tensions that arise between global organizational imperatives and the specific needs of partnering organizations. The analysis will cut across eight domains: environmental context and existing arrangements; governance and structure; models of care; organizational culture; financial and payment structures; consumer awareness, representation, and needs, including the needs of vulnerable populations; systems to measure quality and cost; and progress toward goals. Deliverables include: (1) an interim findings report and a final case-study report on each CCO; (2) eight "rapid-cycle" reports, four for each CCO, that highlight successes and opportunities for improvement and will be presented to the CCOs over the life of the project; (3) presentations and articles for an academic audience; and (4) news releases and reports to policymakers.
