The Foundation's Roadmaps to Health Community grants initiative was designed to identify, support, and disseminate models of cross-sector coalitions working to improve health by taking action to address the social and economic determinants of health, such as education, employment, income, community safety, and family and social support.Pikes Peak Area Council of Governments seeks to establish a coordinated, collaborative system of care for service members, veterans, and their families. The Peak Military Care Network will be housed in a central physical location and will seek to systematize, link, and coordinate civilian, military and veteran services systems. In order to accomplish this they will build a leadership infrastructure that bridges service delivery systems (military, VA, community-based), build a communication and coordination network among providers, develop an information and client navigation system to improve service access and use, design a data collection system, and a plan to monitor and evaluate the system. No RWJF dollars will be used for lobbying. Deliverables will include: a roster of board members, written bylaws, quarterly board training, written membership guidelines, a directory of participating service providers, a data collection system, an improved website, a communications plan, and outreach materials.
