Robert Wood Johnson Foundation Community Health Leaders

The Robert Wood Johnson Foundation Community Health Leaders program was designed to provide recognition for the contributions community health leaders make to achieving RWJF's mission and goals and to enhance the capacity of these individuals to have a more permanent and widespread impact on health care problems.The Community Health Leaders program annually honors 10 individuals who have made significant contributions to improving the health and health care of their communities. Each Community Health Leader (CHL) receives an award in the amount of $125,000. Of that amount, $20,000 goes directly to the leader to recognize past accomplishments and $105,000 supports a project that continues and enhances the leader's work through an appropriate organization with which the leader is associated. Dr. Zane Gates received the 2011 CHL Award and will work with Altoona Regional Health System (AHRS) to implement his project through collaborations with Partnering for Health Services Clinic and the Gloria Gates Memorial Foundation.The focus of the Leader's project is to integrate prevention programs and behavioral health services into primary care settings for high-risk children and adults. Trained staff at Partnering for Health Services Clinic will administer an evidence-based, mental and behavioral health self-assessment screening tool for up to 400 working poor patients. Depending on the results of the screening, patients will be referred to treatment and services by the Altoona Regional Health System Behavioral Health Collaboration or other partner agencies. In addition, the project will support the integration of behavioral health programming for underserved children who are at high-risk for emotional and behavioral health problems at an established after-school program administered by the Gloria Gates Memorial Foundation. It is expected that a mental health therapist will identify and refer up to 50 children to participate in the Good Behavior Game, designed to show positive social interactions with peers and decrease behavioral problems; and the Equine Assisted Learning program to improve coping skills and self-conceptualization.
